Summary:
The Behavioral Health Registered Nurse II (RN II) provides comprehensive psychiatric nursing care to adult and/or geriatric behavioral health patients in an acute inpatient setting. This role is responsible for conducting initial and ongoing psychiatric nursing assessments, administering medications (including psychotropic medications), supporting crisis stabilization, monitoring behavioral and physical health status, and collaborating with the multidisciplinary treatment team to support recovery-focused patient care. The RN II serves as a leader on the unit, ensuring patient and staff safety through continuous environmental awareness, de-escalation practices, therapeutic engagement, and adherence to regulatory and organizational standards.
Responsibilities:
Patient Care & Clinical Assessment Performs comprehensive psychiatric nursing assessments to determine mental status, risk level, clinical symptoms, withdrawal needs, and behavioral functioning. Synthesizes psychiatric, medical, and psychosocial data to inform clinical decision-making and plan of care. Monitors and evaluates patient behavior, affect, safety risks, and changes in clinical condition, initiating appropriate interventions. Establishes and updates individualized nursing care plans based on assessment findings and patient response. Ensures compliance with observation levels, including timely 15-minute checks and accurate documentation. Escalates changes in patient condition and collaborates with providers to adjust treatment plans.
Medication Administration & Clinical Management Administers medications safely, including psychotropic and PRN medications, in accordance with provider orders, clinical guidelines, and facility protocols. Evaluates medication effectiveness and monitors for side effects or adverse reactions, initiating appropriate follow-up. Provides patient education regarding medications and treatment adherence. Ensures accurate, timely documentation of medication administration and patient response.
Therapeutic Engagement & Crisis Intervention Utilizes trauma-informed, recovery-oriented communication and therapeutic techniques to engage patients. Leads and supports de-escalation efforts and crisis intervention using approved behavioral health models. Participates in and may lead therapeutic groups, psychoeducation, and community meetings. Promotes patient participation in treatment planning and recovery goals.
Safety & Compliance Oversees patient safety through observation monitoring, rounding compliance, and milieu management. Directs and participates in the safe application, monitoring, and documentation of restraint or seclusion interventions when clinically necessary, ensuring compliance with regulatory standards. Identifies and mitigates environmental risks and responds promptly to safety concerns or behavioral escalations. Ensures adherence to hospital policies, CMS regulations, DNV standards, and state requirements.
Collaboration & Communication Provides clinical leadership and guidance to LPNs, BHTs, and support staff, including delegation and oversight of care. Collaborates with psychiatrists, nurse practitioners, therapists, case management, and nursing leadership to ensure coordinated, patient-centered care. Participates actively in interdisciplinary treatment team meetings and contributes to care planning and discharge planning. Ensures documentation meets organizational, regulatory (CMS/DNV), and legal standards. Maintains therapeutic communication with patients, families, and care teams while upholding HIPAA and professional boundaries.
Professionalism & Accountability Demonstrates accountability for clinical practice, decision-making, and patient outcomes. Participates in staff huddles, quality initiatives, and continuing education to support clinical excellence. Serves as a role model for professionalism, teamwork, and adherence to standards of care. Supports unit operations, including admissions, discharges, and patient flow. Performs additional duties as assigned.

* ASSOCIATE'S DEGREE
Other information:
REQUIRED: 2+ years related experience
Associate's degree or equivalent in Nursing
Current Arizona state or compact state licensure as an RN
BLS (AHA)
Preferred:
Bachelor of Science in Nursing (BSN) preferred.
Experience in behavioral health, psychiatric, or acute inpatient setting.
Crisis Prevention/Intervention (CPI) or equivalent certification.
